Wake Forest Baptist Medical Center in Winston-Salem is seeking an Ambulatory Infusion Pharmacist to float across nine clinic locations. The role is full-time, 40 hours per week, with Mon-Fri variable hours in an ambulatory infusion setting. Requires Pharm.D.

or BS in Pharmacy and NC licensure, with immunization certification as applicable. You will oversee patient care, counsel patients, and collaborate with the health team to ensure safe, effective pharmacotherapy and high-quality outcomes

Formación

• Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Pharm.D. is required.
• Current state pharmacist license in good standing (North Carolina) is required.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsabilidades

• Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Provide patient education and counseling on medication therapy, adverse effects, compliance, handling, and administration.
• Design, monitor, and evaluate patient-specific pharmacotherapeutic regimens based on biomedical literature.
• Implement recommendations and document in required formats.
• Interpret and review medication orders and prescriptions accurately.
• Support management of medical emergencies when applicable.
• Contribute to medication safety and quality improvement activities.
