Job summary

Advocate Health in Winston-Salem seeks an ambulatory infusion pharmacist to support nine clinic locations. The role requires a Pharm.D. or Pharm.B.S.

Qualifications

• Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) is required.
• Current state pharmacist license in good standing through North Carolina Board of Pharmacy is required.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Provide patient education on medication therapy, adverse effects, compliance, handling and administration.
• Design, monitor and evaluate patient-specific pharmacotherapeutic regimens based on literature and policies.
