Qualifications

• The Staff Nurse practices at a high level of autonomy and accountability in collaboration with the multidisciplinary care team.
• They provide a continuum of patient care across prevention, detection, diagnosis, treatment, follow-up and survivorship.
• They educate patients and families and coordinate care with multiple services.

Responsibilities

• Serves as a point of contact with patients at home and in the ambulatory/outpatient setting to ensure optimal clinical management with the care team.
• Communicates with patients via daily telephone triage and Gateway messages.
• Coordinates home services including visiting nursing, palliative care, hospice, and laboratory services.
