Job summary

Yale NewHaven Health is seeking an entry-level clinical pharmacist to support our Pharmacy Department in Connecticut. You will review medication orders, ensure safety, and work with the healthcare team to optimize patient outcomes.

Qualifications

  • Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
  • Hospital pharmacy experience desirable, but not required.
  • Current Connecticut Pharmacist License, or eligible for licensure in Connecticut.

Responsibilities

  • Demonstrate CHAMP behaviors and promote patient safety.
  • Conduct timely, accurate medication order review that considers patient characteristics including labs and vitals.
  • Demonstrate competency in pharmacy automation and technology as applicable to work area.
  • Adhere to all departmental policies, procedures, protocols, and guidelines.
  • Provide education and training to other pharmacy staff, pharmacy students and residents, and health care team members.
  • Contribute to departmental cost-reduction and revenue generation.

Qualifications

EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree

EXPERIENCE

Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. Hospital pharmacy experience is preferred.

LICENSURE

Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut

SPECIAL SKILLS

This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
