Job summary

Yale New Haven Health is seeking an entry-level clinical pharmacist to join our Department of Pharmacy. You will support CHAMP behaviors, medication management, and patient-centered care as part of a high-reliability organization.

Responsibilities include timely medication order reviews considering labs and vital signs, safe preparation and distribution using pharmacy technology, and education for staff and residents. Connecticut licensure or eligibility is required.

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• ASHP-accredited PGY1 residency or equivalent experience is desirable, but not required.
• Hospital pharmacy experience is preferred.
• Current Connecticut Pharmacist License, or eligible for licensure in CT.

Responsibilities

• Demonstrate CHAMP behaviors and provide excellent customer service quality.
• Review medication orders promptly, considering labs and vital signs for patient care.
• Demonstrate competency in pharmacy automation and technology in the work area.
• Adhere to departmental policies, procedures, and guidelines for safety and quality.

• Contribute to departmental cost-reduction and revenue generation.

Qualifications

EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree

Experience

Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. Hospital pharmacy experience is preferred.

LICENSURE

Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut

Special Skills

This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
