Job summary

Yale New Haven Health seeks an entry-level clinical pharmacist to join the Department of Pharmacy, supporting CHAMP values and patient-centered care within a high reliability organization. You will review medication orders and participate in safety, distribution, and education across the care team. Ideal candidates hold a B.S.

or PharmD from an accredited school and are eligible for Connecticut licensure. Residency experience is desirable but not required; hospital experience is preferred.

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• ASHP-accredited PGY1 residency or equivalent experience is desirable, but not required.
• Hospital pharmacy experience preferred.
• Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut.

Responsibilities

• Demonstrate and promote CHAMP behaviors and prioritise patient safety.
• Conduct timely, accurate medication order review considering patient characteristics, labs and vital signs.
• Demonstrate competency in pharmacy automation and technology as applicable to the work area.
• Adhere to all departmental policies, procedures, protocols, and guidelines.
• Provide education and training to other pharmacy staff, pharmacy students and residents, and health care team members.
• Contribute to departmental cost-reduction and revenue generation.

Special Skills

• This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
