Job summary

Yale New Haven Health is seeking an entry-level clinical pharmacist to join our Pharmacy team in Connecticut. The role supports the organization’s CHAMP patient-safety culture and focuses on medication management, preparation, distribution, and education within a hospital setting.

The ideal candidate has a PharmD or B.S. in Pharmacy and is eligible for CT licensure; ASHP PGY1 experience is desirable but not required. Excellent communication and teamwork are essential.

Qualifications

  • Graduation from an accredited College of Pharmacy with a B.S. or PharmD.
  • Currently licensed or eligible for CT pharmacist licensure.
  • ASHP-accredited PGY1 residency is desirable but not required.

Responsibilities

  • Demonstrate CHAMP behaviors and promote patient safety.
  • Review medication orders considering patient characteristics including labs and vitals.
  • Operate pharmacy automation and technology in the work area.
  • Adhere to departmental policies, procedures, protocols, and guidelines.
  • Provide education and training to pharmacy staff, students and residents.
  • Contribute to departmental cost-reduction and revenue generation.

This is an entry level clinical pharmacist position. The clinical pharmacist supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates competency in all basic clinical pharmacist skills.

Responsibilities
  • Customer Service Quality
    • Demonstrate and promote CHAMP behaviors
  • Medication Management
    • Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signs
  • Medication Preparation and Distribution
    • Demonstrate competency in pharmacy automation and technology as applicable to work area
  • Medication Safety, Regulatory Affairs, and Quality Assurance
    • Adhere to all departmental policies, procedures, protocols, and guidelines

Qualifications

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree

Experience

Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. Hospital pharmacy experience is preferred.

Licensure

Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut

Special Skills

This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
