• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable; hospital pharmacy experience is preferred.
• Current Connecticut Pharmacist License, or eligible for licensure in Connecticut.
• Excellent verbal and written communication skills; ability to work with others.

Responsibilities

• Promote CHAMP behaviors and provide high-quality customer service.
• Conduct timely medication order reviews considering patient characteristics including labs and vital signs.
• Demonstrate pharmacy automation and technology competencies in the work area.
• Adhere to departmental policies, procedures, protocols, and guidelines to ensure medication safety and quality.
• Provide education and training to pharmacy staff, students, residents, and health care team members.
• Contribute to departmental cost-reduction and revenue generation.

Qualifications

EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree

EXPERIENCE

Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. Hospital pharmacy experience is preferred.

LICENSURE

Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut

SPECIAL SKILLS

This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
