Qualifications

• Active District of Columbia RN license or eligibility.
• Minimum of two years of RN experience.
• Ambulatory/primary care or care coordination experience preferred.
• Experience in patient education, triage, and care coordination preferred.

Responsibilities

• Provide direct, patient-facing nursing care in ambulatory clinics.
• Collaborate with physicians and APPs to support safe patient care.
• Perform nursing assessments and document history and symptoms.
• Identify changes in patient condition and escalate appropriately.
• Support clinic with phone/electronic triage and patient communications.
• Administer medications and treatments per policy.
• Document in the electronic medical record accurately and timely.
• Educate patients and families during or after visits.
