Job summary

Regional One Health is seeking a Care Coordination support role to identify and facilitate Gaps in Care (GIC) for patients requiring enhanced care coordination. You will aid the Care Team in tracking high-quality health services, coordinating referrals, and updating patient records within the EHR.

Responsibilities include appointment outreach, data aggregation, and assisting with multi-disciplinary care plans.

Qualifications

• Completion of an approved Medical Assistant program.
• Minimum 1 year experience in a healthcare setting.
• BCLS - Basic Life Support required at hire.

Responsibilities

• Identify and facilitate Gaps in Care (GIC) services for patients needing enhanced Care Coordination.
• Assist with data collection, aggregation and analysis activities.
• Perform appointment outreach and facilitate referrals.
• Update patient information in medical records and ensure timely EHR documentation.
• Track patients as they receive services outside the practice and follow up after ED visits or hospital discharges.
