Baptist Health in Jacksonville, FL is seeking an Ambulatory Care Coordinator to join our team on a full-time days schedule, Monday through Friday, at Metro Square.

You will collaborate with patients and families on advance care planning, align care with goals, and document thoroughly while supporting the care team in a compassionate, patient-centered approach. The role requires ASN and BLS, and 1–2 years in case management with 1–3 years RN hospital experience.

Formación

• 1–2 years Case Management experience.
• 1–3 years RN hospital experience.
• ASN and BLS required.

Responsabilidades

• Collaborating with patient and their families to facilitate discussions around advance care planning.
• Engage with patients to understand their end-of life goals and preferences.
• Work closely with the healthcare team to develop personalized plan of care that aligns with the patient's wishes and medical needs.
• Maintain a compassionate and patient-centered approach, providing emotional support and guidance to patients and their families during what can be a challenging time.
• Accurate documentation.
