Baptist Health in Jacksonville is seeking a Care Coordination Assistant for the Metro campus. This full-time, day-shift role provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care.

Under supervision of licensed staff, you will assist with care coordination, discharge planning, and smooth transitions for patients and families, while maintaining documentation and coordinating referrals and follow-up

Formación

• High School Diploma required.
• Basic Life Support (BLS) certification.
• Driver's License required.

Responsabilidades

• Provide non-clinical and clerical support to the Care Coordination and Pediatric Social Work teams.
• Assist with coordination of care and discharge planning activities.
• Support patient and family care transitions across the continuum of care.
• Communicate with internal departments and external agencies as directed.
• Maintain accurate documentation and assist with data entry in the electronic medical record.
• Coordinate meetings, referrals, and follow-up appointments as needed.
• Ensure timely completion of assigned administrative tasks.
