Baptist Health in Jacksonville, FL is seeking a Care Coordination Assistant for our Care Management team at Baptist South. The role is largely clerical but includes moving around the hospital to deliver Medicare documents and supporting RN Case Managers and Social Workers.

Responsibilities include coordinating care across inpatient, outpatient, and post-acute settings to ensure appropriate use of resources and smooth transitions for patients and families.

Formación

• High School Diploma required
• Basic Life Support (BLS) Required
• Certified Clinical Medical Assistant (CCMA) Preferred
• Certified Medical Assistant (CMA) Preferred
• Registered Medical Assistant (RMA) Preferred
• National Certified Medical Assistant (NCMA) Preferred
• Driver's License required

Baptist Health is looking to add a Care Coordination Assistant in our Care Management department at Baptist Medical Center South. This is a Full Time Days opportunity. The role of the Care Coordination Assistant will be mostly clerical, but they will also be moving around the hospital to deliver a Medicare document to obtain a signature and assisting the RN Case Managers/Social Workers with any needs they might have.
