Job summary

Baptist Health is seeking a Care Coordination Assistant for our Baptist Metro campus in Jacksonville, FL. This full-time, day shift position provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care.

You will assist with discharge planning, care transitions, and communication with internal departments and external agencies. Excellent documentation and data-entry skills are essential in this role.

Qualifications

• High School Diploma required.
• Basic Life Support (BLS)
• Driver's License

Responsibilities

• Provide non-clinical and clerical support to the Care Coordination and Pediatric Social Work teams.
• Assist with coordination of care and discharge planning activities.
• Support patient and family care transitions across the continuum of care.
• Communicate with internal departments and external agencies as directed.
• Maintain accurate documentation and assist with data entry in the electronic medical record.
• Coordinate meetings, referrals, and follow-up appointments as needed.
• Ensure timely completion of assigned administrative tasks.

Under the direction of a licensed professional, the Care Coordination Assistant provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care. This role plays an important part in supporting smooth care transitions for patients and families.
