Baptist Health (Jacksonville) seeks a Care Coordination Assistant to support the Care Management team at Baptist South. The role is primarily clerical but involves moving around the hospital to deliver Medicare documents for signatures and assisting RN Case Managers and Social Workers.

Under supervision, you will coordinate care across inpatient, outpatient, post-acute and community settings to ensure appropriate utilization and smooth transitions for patients and families.

Formación

• 1–2 years hospital experience required.
• High School Diploma required; BLS certification preferred.
• CCMA/CMA/RMA/NCMA credentials preferred.

Responsabilidades

• Under the direction of a licensed professional, provides clinical and clerical support to the Care Coordination Team.
• Coordinate care across the continuum (inpatient/outpatient/post-acute and community) to assure appropriate utilization of clinical and community resources.
• Deliver Medicare documents to obtain signatures and assist RN Case Managers/Social Workers with needs as they arise.
• Provide patient-centered care for coordinating care transitions of patients and families.
