Baptist Health’s Metro campus in Jacksonville, FL seeks a Care Coordination Assistant to support Pediatric Social Work and Care Coordination teams in delivering patient-centered care.

This full-time, day-shift role provides non-clinical and clerical support, coordinates care and discharge planning, and facilitates smooth care transitions with internal and external partners. A High School Diploma and 1–2 years of hospital experience are required, along with BLS and a driver's license.

• Provide non-clinical and clerical support to the Care Coordination and Pediatric Social Work teams.
• Assist with coordination of care and discharge planning activities.
• Support patient and family care transitions across the continuum of care.
• Communicate with internal departments and external agencies as directed.
• Maintain accurate documentation and assist with data entry in the electronic medical record.
• Coordinate meetings, referrals, and follow-up appointments as needed.
• Ensure timely completion of assigned administrative tasks.

Under the direction of a licensed professional, the Care Coordination Assistant provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care. This role plays an important part in supporting smooth care transitions for patients and families.
