Job summary

Confluence Health in Washington state seeks a Clinical Pharmacist to manage medication therapy within the pharmacist scope of practice. You will educate patients and coordinate care with physicians and the care team to optimize therapy and outcomes.

Responsibilities include monitoring therapy, documenting in the EMR, and participating in credentialing, education, and quality initiatives. CPR certification and licensure are required.

Qualifications

• Bachelor's Degree in Pharmacy and licensure in Washington.
• Doctor of Pharmacy degree from an accredited school.
• CPR certification within 90 days of hire.
• Strong computer skills and ability to work in multiple systems.

Responsibilities

• Monitors medication therapy within scope of practice per state law.
• Interprets medical information and adjusts therapy per guidelines.
• Performs ongoing assessment of patient risk factors and therapy indications.
• Documents in the electronic medical record and communicates with care team.
• Coordinate follow-up with patients and other health care providers.
