Job summary

Confluence Health in Washington state seeks a Pharmacist to provide clinical and operational pharmacy services to each patient within their area of responsibility. The pharmacist conducts therapeutic assessment, interventions, and plans for individuals and patient populations as part of the Interdisciplinary Care Team.

This role requires maintaining active WA licensure, CPT coding competence, and collaboration with physicians, nurses, and staff to optimize outcomes and safety.

Qualifications

• Bachelor's Degree (Bachelor of Science) in Pharmacy (RPh).
• Three years of experience as a clinical pharmacist in the ambulatory setting OR residency with ambulatory care experience.
• Licensed to practice in the state of Washington prior to start date.

Responsibilities

• Manage medication therapy and order appropriate labs per guidelines and patient parameters.
• Enter accompanying progress notes with therapy decisions.
• Comply with established protocols for payment conditions as needed.
• Maintain CPT coding competence and pass compliance audits.
• Communicate with patients, families, and other health care providers as needed.
• Support formulary management and pharmacotherapeutics initiatives.
• Participate in department and organizational performance initiatives.
• Provide supervision/oversight of clinical staff.
• Instruct and coach staff, students, and residents.
• Report adverse drug events and mitigate safety risks.
• Maintain active credentialing status and CPR certification.
