Job summary

Confluence Health in Washington state is seeking a Pharmacist to provide clinical and operational pharmacy services to the highest standard for patients in outpatient and hospice settings. The role focuses on therapeutic assessment, interventions, and care plans within an interdisciplinary team.

You will collaborate with Hospice physicians and nurses, ensure safe and cost-effective drug use, and maintain credentialing and confidentiality while supporting home infusion services and ongoing

Qualifications

• Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited school.
• Licensed to practice pharmacy in Washington before start date.
• CPR certification within 90 days of hire.
• Strong computer skills across multiple systems.

Responsibilities

• Ensure safe, effective, and cost-efficient drug use for patients within the hospice program.
• Collaborate with Hospice physicians and nurses for ongoing patient care and therapy optimization.
• Document in the Hospice EMR and maintain credentialing status.
• Coordinate drug distribution and support home IV therapy services.
