Job summary

Confluence Health in Washington state seeks an experienced clinical pharmacist to manage medication therapy and educate patients within the pharmacist scope of practice. You will be part of an integrated care team, coordinating with physicians and other providers to ensure high-quality patient care.

The role requires licensure in Washington, CPR certification within 90 days, and a strong background in ambulatory or clinical pharmacy with CPT coding proficiency.

Qualifications

• Bachelor's Degree of Science in Pharmacy (RPh) required.
• PhD - Doctor of Pharmacy degree from an accredited school of pharmacy.
• Licensed to practice in the state of Washington prior to start date.
• Basic Life Support (CPR) certification within 90 days of hire.
• Strong computer skills, ability to work in multiple systems at the same time.

Responsibilities

• Monitors medication therapy within the scope of practice as outlined in State law.
• Interprets medical records and testing results to adjust therapy per guidelines.
• Performs ongoing assessment of patient risk factors and optimizes regimens.
• Maintains CPT coding competence and passes compliance audits.
• Documents in the EMR, including patient care activities and teaching.
• Communicates with patients, families, and other health care providers.
• Coordinate follow-up with patients and providers.
• Maintain credentialed status via delegated credentialing.
