Job summary

Confluence Health is seeking a Pharmacist to provide clinical and operational pharmacy services with a focus on improving patient outcomes within ambulatory care. You will assess therapy, intervene as needed, and plan care for individual patients and populations as part of the interdisciplinary team.

The role reports to the Pharmacy Manager and requires maintaining active licensure in Washington, along with CPR certification and ongoing participation in department-wide quality initiatives.

Qualifications

• Bachelor's Degree in Pharmacy (RPh) and licensed to practice in Washington.
• Three years of clinical pharmacist experience in ambulatory settings or completion of a practice residency with ambulatory care experience.
• Licensed to practice in the state of Washington prior to start date.

Responsibilities

• Manage medication therapy and order labs per guidelines and patient parameters.
• Enter progress notes documenting MTM decisions.
• Follow established protocols for orders and ensure payment conditions are met.
• Maintain CPT coding competence and pass compliance audits.
• Collaborate with patients, families and healthcare providers to promote coordinated care.
• Advocate for formulary management and participate in P&T initiatives.
