Job summary

Qualifications

• Education: Doctor of Pharmacy.
• License: Current license with the state Board of Pharmacy.
• Certifications: BCLS required; ACLS preferred.
• Skills: Critical thinking, decisive judgment, and ability to work with minimal supervision in stressful environments.
• Rapid learning and navigation of multiple pharmacy/clinical software systems.
• Effective English communication (read, write, speak, and understand).
• Ability to complete the Skills Competency Checklist within 90 days of hire.

Responsibilities

• Coordinates and provides pharmaceutical care for patients as part of an interprofessional team.
• Reviews, interprets, and evaluates medication orders to prevent and resolve drug-related problems.
• Provides drug information and education to medical staff, patients, and pharmacy students.
• Documents interventions and outcomes for drug therapy quality improvement.
• Verifies medication orders for accuracy and checks medications prepared by technicians.
• Manages patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
• Participates in performance improvement and continuing education.
• Performs other duties as assigned.
