Job summary

Central Montana Med Ctr in Lewistown, Montana is seeking a licensed Physical Therapist for a 13-week assignment. You will review physician orders for Home Health and Hospice, conduct evaluations, and develop written treatment plans with short- and long-term goals.

Responsibilities include delivering skilled PT interventions, assessing home safety, documenting progress, educating patients and families, and traveling to patient homes. MT licensure, driver's license and BLS required.

Qualifications

• Current MT PT license and valid driver's license.
• Current BLS certification.
• Ability to travel to patient homes for therapy sessions.

Responsibilities

• Receive and review physician orders for Home Health and Hospice patients.
• Complete comprehensive patient evaluations and establish treatment plans with short- and long-term goals.
• Provide skilled PT interventions including exercises, mobility training, and balance activities.
• Assess home safety and recommend equipment or modifications.
• Monitor progress and adjust treatment plans to achieve outcomes.
• Educate patients and families on plans, home exercises, safety and outcomes.
• Maintain clinical documentation including evaluations, notes and discharge summaries.
• Collaborate with physicians, nurses and therapists involved in care.
• Travel to patient homes and other care settings to deliver services.
• Maintain MT PT licensure, valid driver's license, and BLS certification.

Schedule: 8:00AM - 5:00PM x 5

Term: 13-week assignment with the potential for extension

Pay Rate: $61 - $66/hr

Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in Lewistown, Montana. These positions are 13-week contracts with the ability to extend.

Responsibilities for a PT:

• Receive and review physician therapy orders for Home Health and Hospice patients
• Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-term goals
• Provide skilled physical therapy interventions, including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
• Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
• Educate patients, caregivers, and family members regarding treatment plans, home exercise programs, safety measures, and expected outcomes
• Maintain accurate and timely clinical documentation, including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation
• Collaborate and communicate effectively with physicians, nurses, therapists, and other healthcare professionals involved in patient care
• Travel to patient homes and other care settings to provide therapy services
• Maintain current PT licensure in the state of Montana
• Maintain a valid driver's license
• Maintain current BLS certification
