Job summary

Circadia Health is hiring a Nurse Practitioner to work on-site, seeing patients first, and acting on AI-driven health signals in real time at Skilled Nursing Facilities around Montebello, CA.

You will train facility staff to integrate Circadia into clinical workflows, perform direct patient care, and collaborate with facility teams to improve outcomes. Expect travel within a 30–40 mile radius with mileage covered.

Qualifications

• Certified and licensed Nurse Practitioner in MI.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MI driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

Responsibilities

• Direct patient care: initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day.
• High-volume rounding: Visit SNFs (≈25+ visits/day) focusing on new admits and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration.
• Mentor & collaborate: Support NP onboarding and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30-40-mile radius; mileage and lodging per policy.
