Job summary

Circadia Health in California is seeking a Nurse Practitioner to join our AI-powered patient-care program across SNF and Home Health. You will see patients first, interpret predictive signals in real time, and coach facility teams to intervene earlier and reduce readmissions.

You’ll combine hands-on clinical care with technology-enabled bedside integration, train staff, handle high-volume rounds (25+ visits daily when needed), and cover SNF sites and patient homes within a 30–40 mile radius.

Qualifications

Certified and licensed Nurse Practitioner in CA (ANCC or equivalent).
Eligible for Medicare participation; company bills on your behalf.
2+ years NP experience with complex adult populations; ability to practice independently.
Valid CA driver’s license; comfortable traveling locally.

Responsibilities

Direct patient care in SNF and Home Health AI Care programs.
Round on new admits, facility‑flagged patients, and Circadia alerts.
Train and coach staff at the point of care; integrate Circadia into workflows.
Interpret trend-based insights and drive timely interventions to prevent decline.
Complete documentation within the same day.
Cover SNF sites and patient homes within a 30–40 mile radius.
