Job summary

Circadia Health is seeking a Nurse Practitioner to oversee patient care in SNF and Home Health AI programs. You will see patients first, act on predictive health signals, and coach facility teams to turn data into timely interventions that reduce readmissions.

In SNF facilities you will round on new admits and flagged patients; in home health, extend proactive care at patients' homes. Expect per diem scheduling with 4-5 visits per month and regional travel within a 30-40 mile radius.

Qualifications

Licensed Nurse Practitioner in MI with ANCC certification.
Eligible for Medicare participation; company bills on your behalf.
2+ years NP experience with complex adult populations; able to practice independently.

Responsibilities

Direct patient care: initial and follow-up assessments for flagged residents; adjust plans of care.
Tech bedside integration: train staff to use Circadia in clinical workflows.
Interpret trend-based insights and drive timely interventions to prevent decline.
Complete documentation on the same day; high-volume rounding across SNFs.
Mentor and onboard NP staff; collaborate with clinical ops and product teams.
