Circadia Health in Los Angeles, CA is hiring a Nurse Practitioner to join our SNF and Home Health AI Care programs. You will see patients first, respond to predictive alerts, and coach facility teams to intervene earlier to prevent readmissions.

You'll perform initial and follow-up assessments, integrate AI insights into clinical workflows, train staff at the point of care, and document observations the same day. Travel within a 30–40 mile radius is required.

Qualifikationen

• Licensed CA NP with ANCC or equivalent.
• 2+ years NP experience with complex adults; able to practice independently.
• Eligible for Medicare participation; travel locally required.

Aufgaben

• Direct patient care: initial and follow-up assessments; plan of care; coordinate with facility providers.
• Tech → bedside integration: train and coach staff in clinical workflows.
• Data into action: interpret trend-based insights and drive timely interventions.
• Close the loop: complete documentation the same day.
• High-volume rounding: 25+ visits/day in SNF and 7+ visits/day in home health.
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.
• Regional flexibility: cover SNF sites and homes within 30–40 mile radius.
