Job summary

Circadia Health is seeking a Nurse Practitioner to join our SNF and Home Health AI Care programs. You will see patients first, act on real-time health signals, and coach facility teams to intervene earlier, reducing readmissions.

In SNF settings you’ll round on new admits and flagged patients; in home health you’ll extend care to patients at home, blending hands-on care with AI-enabled workflows, documentation within the same day.

Qualifications

Certified VT NP license and ANCC certification (Adult/Family/Geriatric/Acute).
Eligible for Medicare participation; company bills on your behalf.
2+ years NP experience with complex adult populations; able to practice independently.
Valid VT driver's license; comfortable traveling locally.
Thrives in a dynamic startup environment.
English proficiency required.

Responsibilities

Direct patient care: initial and follow-up assessments; initiate/adjust plans of care; coordinate with facility providers.
Tech bedside integration: train staff and standardize Circadia use in clinical workflows.
Data into action: interpret trend-based insights, prioritize high-risk patients, drive timely interventions.
Close the loop: complete documentation/charts within the same day.
Regional coverage: travel within a 30-40 mile radius; 25+ visits/day where appropriate.
