The Aging Services Case Manager at JFCS provides strengths-based case management and counseling to older adults and families. Services may occur in-office, in-home, or in the community to help clients navigate systems, build coping skills, and access vital supports to age safely and meaningfully.

Responsibilities include assessing needs, developing and evaluating service plans, linking clients to resources, coordinating with partners, and maintaining timely documentation while meeting program

Qualifikationen

• Bachelor’s degree in Social Work, Psychology, Human Services or related field.
• Minimum of two years’ experience working with the aging population preferred.
• Bilingual (English/Spanish) preferred.
• Ability to communicate effectively in writing and verbally.

Aufgaben

• Provide case management services to clients within the Aging Services program.
• Assess client and family strengths, challenges, and needs.
• Develop, write, implement, evaluate, and revise treatment and service plans based on ongoing assessments and client/family input.
• Refer and link clients and families to appropriate internal and external resources.
• Advocate for client needs and coordinate with community agencies/institutions and support systems.
