Job summary

Aging &Long Term Care of Eastern Washington is seeking a Case Manager to deliver advanced case management for older and/or disabled adults in community settings. You will assess needs, develop care plans, coordinate with DSHS and service providers, and ensure compliant service delivery and documentation.

Role requires driving to client homes in Spokane County, maintaining records, and serving as a resource and advocate for clients and families while upholding program regulations.

Qualifications

• Master’s degree in behavior or health science is preferred.
• Bachelor’s degree in behavioral or health sciences and two years of paid social services experience, or four years with a degree and experience.
• Knowledge of aging and long-term care services and policy directives.
• Ability to learn federal/state program regulations and apply them in practice.

Responsibilities

• Provide advanced case management to vulnerable adults in home settings.
• Develop and implement care service plans with coordination of various providers.
• Maintain client records and prepare documentation for hearings when needed.
• Advocate for clients and connect with community resources.
