Job summary

A Case Manager for Aging and Adult Services of Coastal Washington (AASCW) based in Aberdeen, WA oversees assessment, reassessment, and in-home care management for eligible Title XIX clients, coordinating service plans to maximize independence and quality of life.

Collaborating with a multi-agency team, the role ensures funding source requirements are met and advocates as described in the Older Americans Act, while maintaining all program compliance and documentation.

Qualifications

• Master’s Degree in behavioral or health sciences or related field required.
• 1 year paid social service experience with Master’s; or 2 years with BA.
• Or 4 years paid social service experience with BA.

Responsibilities

• Provide assessment and reassessment for frail adults eligible for Title XIX.
• Coordinate in-home care management and service plans to maximize independence.
• Collaborate with multi-agency teams and advocate per Older Americans Act.

Job description

Case Manager for Aging and Adult Services of Coastal Washington (AASCW) based in Aberdeen, WA.

Provides assessment, reassessment and in-home care management services to frail and vulnerable adults who have been found eligible for Title XIX services by the Department of Social & Health Services. The Case Manager collaborates with the client/decision-maker to create a service plan that will maximize independence and quality of life. Coordinates, updates and implements the client service plan within the funding source requirements and agency benchmarks. Collaborates, as part of a multi-agency service delivery team, to provide a wide spectrum of necessary services to the client, including advocacy as described in the Federal Older Americans Act. Maintains all funding source requirements for service delivery to clients.

Master’s Degree in behavioral or health sciences & 1 yr. paid social service experience or BA in similar fields and 2 yrs. paid social service experience; or BA and 4 yrs. paid social service experience.

Requires current WDL & insured vehicle.

40 hrs./wk. Non-Exempt.

$70,453-$92,441 annually
