Job summary

Aging and Adult Services of Coastal Washington (AASCW) in Aberdeen, WA, seeks a Case Manager to provide in-home care management for frail adults eligible for Title XIX services. You will assess and reassess client needs, develop service plans, and collaborate with a multi-agency team to ensure comprehensive care and advocacy.

The role requires a Master’s in behavioral/health sciences or a BA with relevant social service experience, and a valid driver’s license with an insured vehicle.

Qualifications

• Master’s Degree in behavioral or health sciences with 1 year paid social service experience, or BA in related fields with 2 years paid social service experience.

Responsibilities

• Provide assessment, reassessment and in-home care management services to frail and vulnerable adults found eligible for Title XIX services.
• Create service plans with clients to maximize independence and quality of life and implement them within funding source requirements.
• Collaborate with a multi-agency service delivery team to provide necessary services and advocacy as described in the Federal Older Americans Act.
