Job summary

Presbyterian Senior Services (PSS) is seeking a NY Connects Social Worker to coordinate supportive services for older adults in Brooklyn. You will provide information on long-term care options, conduct eligibility screenings, and assist with enrollment in Medicaid, SNAP, and HEAP.

Home visits and client advocacy are key components of this role. The position emphasizes a strength-based, person-centered approach to help clients age in place, with flexible hours and potential remote work 2–3 days

Qualifications

  • Bachelor's degree level education is required; Master's degree in social work preferred.
  • English language proficiency required; bilingual candidates encouraged.
  • Full-time hours from 9am-5pm with occasional evenings and weekends.
  • Experience in health/human/social services, especially with older adults and disabilities.
  • Familiarity with Medicare, Social Security, and NYC HRA is preferred.
  • Proficiency in Microsoft Word, Excel, and Outlook is required.

Responsibilities

  • Provide objective and unbiased information and assistance on long-term care options, services, and supports to community members, professionals, friends, neighbors, and caregivers.
  • Conduct screenings and assessments to determine programmatic and financial eligibility for various programs, both over the phone and in-person.
  • Assist with application and enrollment for publicly funded programs (e.g. Medicaid, SNAP, HEAP) and privately funded programs.
  • Conduct home visits to homebound community members, aiding with applying for benefits and entitlements as deemed appropriate.
  • Utilize a strength-based, person-centered approach to support aging in place, independence, and self-determination.
  • Educate community members on benefits and entitlements to aid decision-making.
  • Advocate for community members navigating governmental and community-based organizations.
  • Support clients with care coordination through various life stages (e.g., return home from facilities).
