Job summary

McGregor PACE in Brooklyn, OH is seeking a Licensed Social Worker (LISW/LSW) to join an interdisciplinary team. You will manage a caseload of 55-62 seniors, conduct semi-annual assessments, and coordinate with nurses and community agencies to support in-home elder care.

The role features a hybrid schedule with 2 days per week remote work, Monday–Friday shifts, and no weekends or holidays. Strong assessment and communication skills are essential, along with reliability and transportation.

Qualifications

• Master's degree in social work and LSW or LISW required.
• Reliable transportation required.
• Must possess strong assessment skills, ability to recognize mental and behavioral problems, and problem-solving skills.

Responsibilities

• Conducts semi/annual assessments of PACE applicants.
• Works with nurse partner to ensure interdisciplinary team has accurate information to assist participant and family members.
• Conducts psychological testing, e.g., Folstein Mini-Mental Exam, Geriatric Depression Scale Index and other cognitive tools, if needed.
• Plans social work interventions; views problem resolution from client benefit perspective.
• Interdisciplinary team support including completion of semi/annual comprehensive assessments, day-to-day documentation, home visits, and care plan updates.
• Communicates with participants, caregivers, and guardians regarding concerns and issues; provides individual/family counseling per plan.
• Corresponds with community agencies that offer services to participants or families, if disenrolled from the PACE program.
• Maintains information on alternative housing services and how to distribute to staff, families/caregivers.
• Demonstrates ability to set priorities to manage caseload effectively with the interdisciplinary team.
• Meets monthly with social work team to discuss problems and issues.
• Identifies participant and family education needs and provides teaching/information.
• Assists in data collection and analysis; participates in performance improvement initiatives.
