Job summary

Metropolitan Area Agency on Aging seeks a Minnesota Aging Pathways Resource Coordinator to support older adults and their families. The role involves assessments, options counseling on long-term care, housing, and financial resources, and coordinating services with health providers and community agencies.

The position is full-time, 40 hours weekly, with a hybrid schedule—in-office two days per week and remote work the rest of the time.

Qualifications

• Bachelor's degree in a human services field with 1 year of related experience.
• Two-year certificate or degree in human services with 3 years of related experience.
• Five years of care coordination/case management experience; knowledge of aging services and resources.

Responsibilities

• Provide person-centered support to older adults and caregivers.
• Conduct assessments and options counseling on long-term care services.
• Coordinate services and referrals with healthcare providers and community agencies.
• Advocate for clients to ensure access to appropriate supports and document plans.
