Northwest Regional Council (NWRC) is hiring a Health Homes Care Coordinator to support clients with chronic conditions by coordinating medical and social services, including transportation, care planning, and referrals. The role blends field work with in-office collaboration to ensure seamless care across providers.

Ideal candidates bring a master’s or bachelor’s in a related field and experience in direct human services, with strong communication and advocacy skills.

Qualificações

• Master's Degree in Behavioral or Health Sciences or related field and 1 year of experience providing direct human services to clients, preferably in a community setting OR
• Bachelor's Degree in an equivalent field and at least 2 years of experience providing direct human services to clients, preferably in a community setting OR
• An equivalent combination of education and experience that provides the skills, knowledge and ability to perform the work

Responsabilidades

• Provide coordination of medical and social services needed, including long-term care, transportation, food services, and referrals.
• Facilitate communication between clients, medical providers, family and other caregivers.
• Help clients navigate services to achieve health goals and maintain recovery.
