Job summary

Easterseals NH is seeking a Community Resource Specialist to support NH residents through the Aging and Disability Resource Center. The role focuses on unbiased, person-centered education, advocacy, assistance, and navigation of long-term services and community resources.

Full-time 40 hours weekly, based in Manchester and Nashua, with a starting wage of $21 per hour. Responsibilities include assessments, referrals, and Medicare information, along with required certifications.

Qualifications

• Bachelor’s degree or equivalent work experience in elderly/disabled programs.
• Familiarity with state/federal long-term care programs (Medicaid/Medicare).
• Strong communication and problem-solving abilities.
• Comfort with home visits and community outreach.

Responsibilities

• Assess client needs and refer to long-term supports and services by phone, email, walk-ins, or home visits.
• Provide information and referrals to community resources and supports.
• Counsel and support Medicare beneficiaries, especially during enrollment periods.
• Complete eligibility screening and assist with applying for programs.
• Document contacts in the assigned database and follow protocol.
• Attend required meetings and perform general office tasks as needed.
• Obtain mandatory certifications within 6–12 months of hire.
