Walworth County seeks a dedicated professional to provide information, counseling, eligibility determinations, and advocacy for older adults, adults with disabilities, and youth. Responsibilities include case management, service coordination, and maintaining necessary documentation within state and county guidelines.

The role requires a Bachelor's in social work or related field, with at least one year of relevant experience; a Master's is preferred.

Formación

• Bachelor's degree in social work or related field and at least one year of relevant prior experience.
• Master's degree preferred and experience with aging and disabled populations.

Responsabilidades

• Facilitate consumer access to services and monitor purchased services.
• Evaluate individuals and coordinate service plans and treatment.
• Administer functional and financial screens to determine eligibility.
• Provide short-term case management including in-home contacts.
• Assist with annual protective reviews for long-term care enrollees.
