Job summary

National Church Residences Alliance Towers in Alliance, OH is seeking a Service Coordinator I to coordinate programs and services for residents, helping them maintain a good quality of life and age in place. Requirements include a Bachelor’s degree in social service or related field and 3–5 years’ experience working with elderly and disabled populations, with proficiency in Windows and Microsoft Office.

Qualifications

Bachelor’s degree in social service or related field and 3–5 years’ experience with elderly/disabled populations.
Excellent communication and interpersonal skills; ability to engage residents and partners.
Proficiency with Windows and Microsoft Office; basic documentation and reporting skills.

Responsibilities

Coordinate programs and activities for eligible residents and liaise with community agencies.
Outreach to participants, assess needs, and refer to community services following program requirements.
Document services, maintain care plans, and monitor participant progress and referrals.
Maintain professional boundaries and build informal support networks for residents.
