Job summary

Jobtailor is seeking a qualified care coordinator/case manager to support a caseload of aging and disabled clients in the New York City area. You will conduct intakes, perform regular assessments, and monitor progress through in-person or virtual visits, while coordinating medical decisions with a multidisciplinary team.

The role requires strong interpersonal and organizational skills, flexibility, and the ability to travel across the five boroughs.

Qualifications

• Strong interpersonal and communication skills are essential.
• Experience in care coordination/case management preferred.
• Flexibility, patience, and follow-through are required.

Responsibilities

• Coordinate care and manage a caseload of 20–25 aging or disabled clients.
• Conduct client intakes and monthly to annual assessments.
• Monitor client conditions via monthly in-person or virtual visits and calls.
• Coordinate medical and end-of-life decisions with medical staff and legal team.
• Advocate for clients with agencies and institutions.
• Collaborate with families and loved ones.
• Attend team meetings and case reviews.
• Initiate and coordinate prepaid funeral arrangements.
• Maintain communication with medical and home care providers.
• Arrange home cleaning and extermination when needed.
• Keep timely, detailed case notes and records.
• Prepare initial, annual, and final accounting reports.
• Prepare admin docs and reports for grants.
• Organize and schedule work in Outlook calendar.
• Update electronic client records.
• Screen clients for government and private benefits.
• Gather information and submit applications and recertifications.
• Apply for and maintain clients’ medical insurance.
• Travel across NYC boroughs and possibly other regions to meet clients/families.
• Travel to other regions may be required.
• Bachelor’s/Master’s in Social Work preferred.
• Authorization to work in the United States.
