Job summary

CARES in New York, NY seeks a Family Support and Care Manager to coordinate services for client/family goals and manage a multi-family caseload. The role requires in-person work in Manhattan with travel; hybrid option may develop after training and caseload full.

Responsibilities include coordinating OPWDD services, assisting Medicaid applications, and liaising with care teams while ensuring regulatory compliance and accurate record-keeping.

Qualifications

• Associate's degree in Psychology, Sociology, Social Work, Human Services or related field.
• Or minimum of 2 years of case management or medical care coordination experience.
• Bilingual in Mandarin and/or Cantonese required.
• Ability to use EHR systems and MS Office to manage records.
• Strong verbal and written communication; highly organized.

Responsibilities

• Coordinate services to meet client/family goals per care plans.
• Obtain OPWDD services and assist Medicaid applications as needed.
• Assist families in transitioning to a CCO for OPWDD.
• Attend weekly in-person staff meetings and maintain records.
