Role Description

The Aging Life Care Manager (nurse) is a part-time, hybrid role based in Northern New Jersey with a mix of in-person visits and work from home for documentation and coordination tasks. This role will also include medication management for some clients, in their homes. Day-to-day responsibilities include conducting client assessments, developing and updating individualized care plans, and coordinating medical, home care, and community services. The role involves ongoing care management, monitoring clients’ well-being, advocating with healthcare providers, and supporting families in decision-making and crisis situations. The Aging Life Care Manager will maintain detailed records, communicate regularly with clients and their support networks, and participate in care conferences and referrals to appropriate resources. The position requires flexibility to travel locally for home visits and appointments, while using remote tools for follow-up, planning, and collaboration.
