Job summary

Westmoreland County Area Agency on Aging is seeking a compassionate, full-time Aging Care Manager II to protect older adults and support their independence within the community. The role involves investigations, information gathering, services coordination, and possible court testimony, with on-site hours and potential standby coverage.

Minimum qualifications include a bachelor’s degree in a social sciences field and six months of relevant experience or an equivalent combination.

Qualifications

• Six months as Aging Care Manager 1 or County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.
• PA residency requirement must be met.

DESCRIPTION OF WORK

This position focuses on protecting older adults and supporting their ability to remain safe within the community. As an Aging Care Manager 2, you will perform the following duties:

• Protective Investigations: Investigate reports involving possible abuse, neglect, exploitation, or abandonment of older adults
• Information Gathering: Conduct interviews and collect details from consumers, families, and professionals to assess safety concerns
• Service Coordination: Work with medical, legal, and social service professionals to reduce risks and support needed interventions

Work Schedule and Additional Information

• Full-time employment
• Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with 30-minute lunch.
• May be required to be on a standby rotation for off-hours coverage.
• Telework: You will not have the option to telework in this position.

QUALIFICATIONS
