Westmoreland County Area Agency on Aging is seeking a full-time Aging Care Manager II to protect older adults and help them remain safe and independent in the community.

This position conducts protective investigations, information gathering, service coordination, emergency response, court participation and ongoing case monitoring; schedule is Monday–Friday 8:30 AM–4:00 PM with no telework option.

Qualifications

• Six months as an Aging Care Manager 1 or a County Caseworker 1
• Successful completion of the County Social Casework Intern program
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field
• An equivalent combination of experience and training

This position focuses on protecting older adults and supporting their ability to remain safe within the community. As an Aging Care Manager 2, you will perform the following duties:

• Protective Investigations:Investigate reports involving possible abuse, neglect, exploitation, or abandonment of older adults
• Information Gathering:Conduct interviews and collect details from consumers, families, and professionals to assess safety concerns
• Service Coordination:Work with medical, legal, and social service professionals to reduce risks and support needed interventions
• Emergency Response:Respond during or after normal work hours to handle urgent protective situations

Work Schedule and Additional Information:

• Full-time employment
• Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with 30-minute lunch.
• May be required to be on a standby rotation for off-hours coverage.
• Telework:You will not have the option to telework in this position.

QUALIFICATIONS

Minimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1;or
• Successful completion of the County Social Casework Intern program;or

• An equivalent combination of experience and training.
