Job summary

UnitedHealth Group is seeking an Afterhours Registered Nurse for Deaconess Hospice (LHC Group) to deliver hospice care after regular hours, including nights and weekends, with a focus on timely assessments and safe home visits.

Primary duties include phone triage, after-hours crisis response, and nursing evaluations within 48 hours of referral, with documentation of care and patient outcomes in diverse home settings. MS-specific experience is preferred and a valid RN license is required.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• MS state requirement: at least one year full-time RN experience; three years FT LPN may substitute for RN experience.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to after-hours crisis calls from patients and families with urgency.
• Perform initial nursing evaluation within 48 hours of referral when assigned.
• Conduct after-hours visits including admission, death, recertification and routine visits.
• Identify patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Provide care in home settings regardless of client background.
