Job summary

Saint Mary's Hospice, part of LHC Group and aligned with UnitedHealth Group, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care after regular business hours, including nights and weekends. You will triage needs by phone, perform evaluations, and conduct after-hours visits to support patients and families.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or reliable public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Preferred: 1+ years of clinical experience.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond to after-hours crisis calls from patients and families with urgency.
• Perform the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct after-hours visits including admissions, death visits, recertification visits and routine visits.
• Identify patient/family physical, psychosocial, emotional, and environmental needs and reassess at least every 15 days.
• Document problems, goals, interventions, and patient/family response to hospice care.
