Job summary

UnitedHealth Group's Northern Arizona Hospice, part of the Optum family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and families after regular business hours, including nights and weekends.

You will triage needs by phone, conduct afterhours visits, perform eligibility assessments within 48 hours of referral, and document care while upholding the dignity and diversity of every patient.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to crisis calls after hours with appropriate urgency.
• Make the initial nursing evaluation within 48 hours of referral.
• Conduct afterhours visits including admissions, death visits, recertification, and routine visits.
• Identify and reassess patient/family needs at least every 15 days.
• Document problems, goals, interventions, and patient/family responses.
• Provide care in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
