Job summary

UnitedHealth Group is seeking an Afterhours Registered Nurse to join our hospice team on a 7-on/7-off schedule. You will provide comprehensive hospice care after regular hours, including nights and weekends, primarily in patients' homes.

You will triage calls, perform eligibility evaluations within 48 hours of referral, and conduct afterhours visits such as admissions, death visits, and recertifications while documenting care plans and patient responses.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transit.
• Preferred CPR certification or ability to obtain within 90 days.

Responsibilities

• Deliver hospice care after hours including at night and on weekends.
• Triage patient needs over the phone and arrange in-person visits.
• Perform initial nursing evaluations to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours visits, including admissions, death, recertification, and routine visits.
• Assess patient/family needs and update care plans every 15 days.
• Document problems, goals, interventions, and responses.
