Job summary

TFI Family Services, Inc. is seeking a full-time Aftercare Case Manager in Kansas to support children and families who have achieved permanency through reintegration, adoption, or guardianship.

This role focuses on risk management and crisis intervention, with an emphasis on coordinating services and ensuring thorough documentation. You will engage with families in-person to support reunification, address concerns, and connect them with community resources while contributing to a supportive team

Qualifications

• Bachelor’s degree in human services is required.
• Valid driver’s license is required.
• We provide training for the role.

A Day in the Life

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
