Job summary

TFI Family Services, Inc. in Kansas (Independence area) is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, or guardianship.

You will focus on risk management, crisis intervention, documentation, and coordinating services, meeting families in person to support reunification. We seek compassionate advocates who connect families with community resources, are eager to learn, and committed to strengthening

Qualifications

• Bachelor’s degree in human services required.
• Valid driver’s license required.
• Willingness to learn—we’ll provide training!

A Day in the Life

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in‑person to support reunification and resolve any concerns.
